Case Details
This patient, a male facial rejuvenation candidate, came to Dr. David Stoker seeking improvement in heavy, low-set brows and excess upper eyelid skin that were affecting the appearance of his upper face. Dr. Stoker performed three coordinated procedures in a single surgical session: a direct brow lift, upper eyelid surgery (upper blepharoplasty), and CO2 laser resurfacing around the eyes. The combination was designed to address brow position, upper eyelid skin excess, and periorbital skin quality together, rather than treating any one component in isolation.
For the brow lift, Dr. Stoker used a direct approach, placing the incision along the natural contour and shadow of the brow itself. This differs from an endoscopic brow lift, which relies on small scalp incisions and internal fixation to elevate the brow from above. Dr. Stoker has moved toward direct brow lift techniques in appropriately selected patients because he has found that the surgical burden of an endoscopic approach can be disproportionate to the amount of lift it actually achieves. A direct incision, by contrast, allows for a more powerful and predictable elevation through a comparatively smaller and more targeted procedure. In this case, the incision removed slightly more than a centimeter of brow height. Because the incision sits directly within the brow, the surrounding brow hairs help camouflage the scar as it matures, an important consideration when deciding whether this technique is appropriate for a given patient's anatomy and skin type.
The upper eyelid surgery addressed a separate but related concern: excess skin along the upper lid that was contributing to a heavy, tired appearance. Dr. Stoker removed slightly more than a centimeter of skin through this second incision, meaning that between the brow and the upper eyelid, nearly an inch of excess skin was addressed across the upper third of the face. Performing both procedures together allowed the brow and eyelid to be balanced against one another, rather than correcting one area in a way that might look disproportionate relative to the other.
CO2 laser resurfacing was then used around the eyes to improve skin quality and texture in the same treatment area. Laser resurfacing works by controlled removal of the outer skin layers, prompting new collagen formation as the skin heals. Some postoperative swelling and puffiness is expected after CO2 laser treatment and typically continues to improve gradually over several months, which is consistent with the early appearance seen in these one-month postoperative photographs.
At one month after surgery, the brow incision is already difficult to distinguish from the patient's natural skin creases, and the brow shows a meaningful, well-supported elevation. Combining a direct brow lift with upper blepharoplasty in this way is particularly effective in appropriately selected male patients, whose thicker brow hair and skin characteristics can support a direct incision while still allowing it to heal inconspicuously. Dr. Stoker has noted that a less direct brow lift approach would likely not have achieved the same degree of correction in this patient, even though it might have involved more extensive surgery overall.
Careful patient selection remains central to this technique. Not every patient is a good candidate for a direct brow incision, and outcomes depend heavily on skin type, brow hair density, and how the individual tends to scar. In this case, those factors supported a confident recommendation for the direct approach, and the early result reflects a brow and upper eyelid that appear proportionate and rested rather than overdone. As healing continues and the residual laser-related puffiness resolves, the transition between the brow, upper lid, and periorbital skin is expected to become even smoother.
