Having completed hundreds of these operations, I have refined my techniques to obtain beautiful results for my patients. Panniculectomy Surgical Technique

In a panniculectomy, I remove the hanging skin and fat from the lower abdomen with a wedge resection. I do not undermine the remaining skin as I would in the typical abdominoplasty (tummy tuck). I also do not tighten the muscles. I then bring the edges of fat and skin together with a layered closure. The advantage of not undermining is that the procedure is smaller and potential problems are fewer. The disadvantage is that there's minimal improvement to the central and upper abdomen.

Candidates for a Panniculectomy

The best candidates for a panniculectomy include those that have:

  • Loose abdominal skin confined primarily to the lower abdomen
  • More weight to lose, but find the lower abdominal skin (pannus) so debilitating that it impedes further weight loss
  • Substantial need for removal of the hanging skin, but medical problems or advanced age that would make a smaller operation desirable

Recovery After Panniculectomy

Most patients take at least one week off from work following a panniculectomy. Patients should begin walking within hours of surgery, but full exercise should not be resumed for approximately 6 weeks.

Risks of Panniculectomy

Possible risks for a panniculectomy include seroma (fluid collection), delayed wound healing, bleeding, infection or venous blood clots. Each of these complications may be treated effectively with prompt medical care.
