Will your stomach go flat after a tummy tuck? The honest answer is that it can, dramatically so, when the right techniques are used. For many patients I consult with about tummy tuck surgery, their goals go beyond simply removing excess skin. They want a flatter, firmer, more sculpted midsection that looks natural and proportionate to the rest of their body. Whether due to changes after pregnancy, significant weight loss, or aging, the abdomen can be one of the most frustrating areas to improve with diet and exercise alone.

That’s why I developed my signature Hourglass Tuck, which is designed to deliver results beyond a flat abdomen. How a Tummy Tuck Creates a Flatter Stomach

A tummy tuck can dramatically refine the waistline and restore abdominal contours, but it’s important to understand exactly how those results are achieved. In my practice, I focus on combining advanced muscle repair, precise skin tightening, and meticulous attention to details like the bellybutton to help patients achieve elegant, balanced results that last.

One of the biggest misconceptions about tummy tuck surgery is that the abdominal muscles are cut or altered during the procedure. In reality, the muscles themselves are not cut.

During pregnancy or weight fluctuations, the fibrous fascia surrounding the abdominal muscles can stretch and separate, leading to a protruding or weakened midsection. This condition, known as diastasis recti, often causes the abdomen to look rounded even in patients who are otherwise fit and healthy.

My technique focuses on tightening this supportive inner layer rather than cutting the muscles themselves. Using secure two-layer sutures, I carefully repair and reinforce the fascia that surrounds the muscles. This restores internal support and helps narrow the waistline, creating a flatter and more toned abdominal contour.

Once the internal repair is complete, I remove excess skin and tighten the remaining skin smoothly across the abdomen. This combination of fascial tightening and skin removal is what creates the dramatic transformation many patients are looking for.

What Makes the Hourglass Tuck Different?

Not all tummy tucks are performed the same way, and a surgeon’s technique has a major influence on both the appearance of the results and the recovery process. The Hourglass Tuck not only creates a flatter abdomen, but also sculpts the entire midsection for a more balanced, feminine silhouette.

I begin by making incisions as low as possible on the abdomen—just above the pubic area—so patients can wear small bikini bottoms, low-cut pants, crop tops, or any other fashion that reveals most of the midsection. 

I then repair diastasis recti by tightening the stretched fascial layer surrounding the abdominal muscles, helping restore core support and create a narrower waistline. Excess skin is removed, and the remaining skin is tightened smoothly.

When appropriate, I also incorporate a mons or pubic lift to create a smoother transition through the lower abdomen and pelvic area. To further enhance overall proportions, I may strategically contour the waist and hips with liposuction and use fat grafting techniques to add subtle volume to the buttocks or breasts.

The Bellybutton’s Importance

The appearance of the bellybutton (umbilicus) following a tummy tuck is a crucial aspect of the procedure, yet it is often overlooked. An unnatural-looking belly button can immediately reveal that someone has had a tummy tuck, even when the abdomen itself looks beautiful.

The techniques used by different plastic surgeons vary quite a bit, and so do the results. My approach focuses on several key principles:

  • Hiding the scar by placing it slightly on the inside edge of the belly button
  • Removing some surrounding fat to recreate youthful contours
  • Using a crescent-shaped skin excision to create a soft, natural upper fold
  • Deepening the base of the umbilicus to create a natural-looking “innie” bellybutton (unless the patient specifically prefers an “outie”)
  • Burying external sutures so stitch marks are not visible after healing

These details may sound subtle, but together they make an enormous difference. The bellybutton should look like it naturally belongs on the abdomen. In my experience, this level of refinement is one of the reasons patients feel especially confident wearing swimsuits, crop tops, or fitted clothing after surgery.

Maintaining Your Flat Stomach

One of the best aspects of a tummy tuck is that the results can be long-lasting. However, maintaining your outcome requires a commitment to keeping your weight stable and leading a healthy lifestyle.

Significant weight gain after surgery can stretch the skin and abdominal tissues again, potentially affecting the results. Pregnancy can also reverse some of the improvements created during muscle and fascial repair, which is why I generally recommend waiting until childbearing is complete before having a tummy tuck.

To preserve your flatter stomach for years to come, I encourage patients to:

  • Maintain a stable weight.
  • Exercise consistently once fully healed.
  • Focus on core strength and overall fitness.
  • Eat a balanced, nutrient-rich diet.
  • Avoid smoking and excessive weight fluctuations.

A tummy tuck isn’t a substitute for a healthy lifestyle, but it can provide a powerful foundation that helps patients feel more motivated and confident moving forward.
