Abdominoplasty Techniques

Abdominoplasty, also called a tummy tuck, provides many women and men with a dramatic improvement in the appearance of their abdominal areas. Most of the abdominoplasty procedures I perform in my practice near Los Angeles include the following elements:

  • Extra skin and fat of the lower abdomen is removed through a lower abdominal incision.
  • Abdominal muscles are tightened, creating a much flatter abdomen.
  • Loose skin of the upper abdomen is then pulled down like a window shade, smoothing the skin and fatty bulges of the abdomen.
  • The belly button is repositioned, and the skin around it is pulled down to a much lower location.

There are different types of tummy tucks depending on patient needs. There's also a large variation in techniques among surgeons. Here, I'll elaborate on each element of my tummy tuck procedures that leads to superior results. Review my Tummy Tuck FAQ page for additional information.

Incisions (Scars)

I use modern plastic surgery techniques to create the best-looking scars possible. The main scar for a tummy tuck is located at the lower abdominal waistline. I design my scars to be particularly low so they'll be concealed when wearing low-cut bathing suits, pants or other clothing. In the 1980's, it was fashionable to have high-cut bathing suits, and many surgeons still place the scars high on the sides. Unfortunately, those scars are visible with today's fashion. I discuss scar position with my patients and plan the incisions to fit their lifestyle. In general, the longer the incision, the more skin may be tightened and removed.


During pregnancy, the rectus abdominis ("six-pack") muscles of the abdomen are stretched apart. For many women, the muscles don't return to their original position after pregnancy. We call this condition diastasis recti. For these patients, I tighten the abdominal muscles without cutting any muscle by using secure two-layer sutures for the fibrous fascia surrounding the muscles. For patients who don't have a diastasis recti, such as young weight-loss patients, it may not be advantageous to tighten the muscles. Those patients have an even faster recovery from the operation.

Belly Button (Umbilicus)

There's a large amount of variability among plastic surgeons in the appearance of the belly button following abdominoplasty. I have my own special technique to accomplish the following goals with the new belly button:

  • Hide the scar by placing it slightly on the inside edge
  • Remove some fat surrounding the umbilicus to recreate youthful contours
  • Use a crescent- shaped skin excision to create a natural appearing upper fold of skin
  • Deepen the base of the umbilicus to create an "innie" belly button (unless an "outie" is requested)
  • Bury the external sutures so that stitch marks are not visible on the tension free closure

Each of these details contributes to creating beautiful belly buttons for my Los Angeles tummy tuck patients.

Pubic Lift

Rejuvenating the pubic area (mons pubis) is an important part of my abdominoplasty procedures. Many surgeons overlook the opportunity to refine the pubic area. I include lifting and tightening of the pubic area as an integral part of the tummy tuck procedure. In my consultations with patients, I'm able to show the dramatic enhancement a pubic lift will add to their tummy tuck result.
